HealthPro Academy stores your name, email, country, professional role, registration time, consent, assessment attempts, scores and certificate references on this hosting account to administer the CSSD Week activity and review certificate proof for the October offer.

Your email and professional details are accessible only to the authorized administrator. Anyone holding your certificate QR link can view your name, score, reference and issue date. No public participant directory is provided.

Records remain until removed by HealthPro Academy. Contact info@healthproacademy.com to request access, correction or deletion. Do not enter patient information. Passing candidates receive their participation certificate by email. Delivery is recorded for activity administration. The activity does not send marketing emails or take payment.

You may retake the assessment without a limit. The certificate confirms participation in an educational activity and does not establish professional competency.
